Short-Term Support for Safe Recovery

Comfort Living Services offers Recuperative Care through Minnesota DHS for individuals who are medically stable but unable to return home after a hospital or clinic discharge. This short-term care bridges the gap between acute medical treatment and stable housing or long-term services.

Who Qualifies for Recuperative Care

We serve individuals who meet DHS-recognized eligibility criteria:

  • Adults being discharged from a hospital, emergency department, or clinic
  • Individuals experiencing homelessness or housing instability
  • Those who are medically stable but need short-term support or help with daily living
  • Persons not requiring inpatient medical care, detox, or 24-hour nursing

Health-Related Support

  • Basic health monitoring and observation

  • Support getting to and from follow-up medical appointments

Daily Living Support

  • Supervised or staffed environment 24 hours a day
  • Assistance with meals, bathing, grooming, and other personal care needs
  • Laundry, cleaning, and maintaining a sanitary space
  • Sleep/rest support in a quiet, recovery-focused setting

Care Coordination & Case Management

  • Discharge planning in partnership with hospitals or clinics
  • Support connecting to long-term care, housing stabilization, or behavioral health
  • Assistance with insurance, benefits, and accessing MA-funded programs
  • Coordination with county case managers, hospitals, and social service providers
