What Is Housing Stabilization Services

Supporting Independence Through Person-Centered Housing Services

Housing Stabilization Services (HSS) is a Minnesota Medical Assistance (MA) benefit designed to help individuals with disabilities and complex needs find and maintain safe, stable housing. At Comfort Living Services, our dedicated team supports clients in overcoming barriers and building the skills needed to live independently in the community.

Who Qualifies for HSS

To be eligible, individuals must:

  • Be enrolled in Medical Assistance (MA)
  • Have a disability or disabling condition (mental health, physical health, substance use disorder, or cognitive challenges)
  • Experience housing instability, such as:
    • Being homeless or at risk of homelessness
    • Transitioning out of an institution or treatment setting
    • Facing eviction or unsafe living conditions

Housing Transition Services

Housing Sustaining Services

  • Ongoing support to maintain stable housing
  • Coaching on tenant rights and responsibilities
  • Conflict resolution with landlords or neighbors
  • Budgeting and household management
  • Coordination with case managers, landlords, and other providers

Housing Consultation (for individuals without a case manager)

  • Development of a person-centered housing plan
  • Help understanding eligibility and service options
  • Guidance on accessing community resources and housing support
