Home and Community-Based Services (HCBS) are Medicaid-funded supports that help people with disabilities and older adults live in their own homes or community settings instead of institutions or nursing homes. Services are person-centered and tailored to individual needs.
Who Qualifies for HCBS
- Must be on Medical Assistance (MA)
- Must meet level-of-care criteria (such as nursing facility level of care)
- Typically includes seniors, people with disabilities, or those with complex medical conditions
- Requires assessment by county or tribal staff
Services Include
- Personal care assistance
- Help with activities of daily living (ADLs)
- Health-related tasks and monitoring
- Behavioral health supports
- Respite care
- Homemaker and chore services
- Residential options such as adult foster care or supported living
