Home and Community-Based Services (HCBS)

Home and Community-Based Services (HCBS) are Medicaid-funded supports that help people with disabilities and older adults live in their own homes or community settings instead of institutions or nursing homes. Services are person-centered and tailored to individual needs.

Who Qualifies for HCBS

  • Must be on Medical Assistance (MA)
  • Must meet level-of-care criteria (such as nursing facility level of care)
  • Typically includes seniors, people with disabilities, or those with complex medical conditions
  • Requires assessment by county or tribal staff

Services Include

  • Personal care assistance
  • Help with activities of daily living (ADLs)
  • Health-related tasks and monitoring
  • Behavioral health supports
  • Respite care
  • Homemaker and chore services
  • Residential options such as adult foster care or supported living
